MSHSAA physical forms

For athletic participation this coming school year, you will need to have a current physical from a doctor. Many doctors will have forms, but MSHSAA also provides one. You can click on the link below to have a copy to provide Tolton Catholic for athletic participation.

Multi-Sport Athlete Form

At Fr. Tolton Catholic High School students have an opportunity to participate in more than one sport in a season. It is essential that we get maximum participation in all of our athletic programs, and being able to participate in multiple sports is a way to achieve this.

2024-25 All Sports Pass

Those who purchase an All Sports Pass will receive a pass (or passes) to attend all home athletic contests in all seasons for the 2024-2025 school year. All money raised through athletic passes stays in the Tolton Catholic Athletic/Activities Department to support our student athletes.
